Saint Elizabeth Community At Home Together Program is an exclusive membership program designed to help RI Seniors maintain their health and independence from the comfort of their own home while also providing financial and wealth protection.
Aging well requires more than simply access to care- it requires foresight, flexibility and partnership. Our At Home Together membership program is designed for individuals who wish to age confidently at home while maintaining their independence, choice and control. The program offers a proactive approach with personalized care and service coordination providing Members peace of mind through life’s transitions.
Our Membership plans are structured to grow with you as you age
Whether you need assistance after a planned outpatient surgery or experience a fall, we will be there for you when you need us. You will have a trusted partner by your side for life- ensuring that you will always have the right level of care, at the right time in a place you call home.

With the right plan, aging in place becomes not just a dream, but an achievable reality. Our Membership plans are structured to grow with you as you age.
How does it work?
Healthy, independent individuals join the program by paying a one-time enrollment fee plus an ongoing monthly fee based on age and the level of service plan selected. In return, Members receive lifelong security, personalized support and predictable access to care.
Saint Elizabeth’s At Home Together program can act as an alternative to long-term care insurance or as a supplement to long-term care insurance, depending on the plan that is selected.
Becoming a Member proactively protects against rising long-term care costs, and helps protect your assets.

Membership Benefits
Our goal is to help you remain healthy so you can live in the home you love. As a Member, you will receive the following wellness-focused services and support:
• 24/7 access to personal care coordination
• Home safety and wellness assessments
• Personalized care plan that meets your needs as you age
• Lifestyle and wellness programs
• Personal Emergency response system
But should the need arise, you have access to the following long term care services up to the daily maximum rate contracted for:
• Personal Care Service/home health aides
• Adult Day services
• Memory Care
• Facility care – Assisted Living, Skilled Nursing
Members have the freedom to age in place with confidence knowing their future care is already planned and secured.
